• Located at a Private Hospital in a North Melbourne suburb
• Working with GEM patients discharged from acute hospital
• Caseload includes ADL assessments with restorative and medical clients with physical and cognitive problems
• To start ASAP (or the earliest date of your convenience)
• Flexibility in days/hours worked (ideally 3 days per week)
• Ongoing for 3-6 months
